- Sports physicals MUST be dated after April 15 of the current year, and MUST be completed prior to August 12th (the first day of mandatory practice!)
- Physicals can be done at your family doctors office, or by contacting the Student Health Center at Springfield Middle School or Central High School

- ALL PLAYERS MUST PAY $35 FOR INSURANCE!
- This is a high school requirement for participation, and is over and above whatever health insurance you might have. It is a supplemental policy that will work with your current health insurance to make sure all bills are paid if your player is injured.
- $35 must be paid to either the coach or the athletic office (OR can be paid online through Final Forms) BEFORE your student can compete in any games!
